Good news: Your treating physician can order one for you. To share more information about Tempus with your physician, we offer the following options:

Tell us who your provider is and we’ll reach out to share more information. If your provider has ordered Tempus testing before, they will be able to submit a new order for you. If they are new to Tempus, the details below will enable us to reach out and add them to our system.

Have an appointment coming up with your physician?

You can download and print the order form so that they can fill it out at your next appointment. Once the form is submitted by your provider, it can be sent back to Tempus via Fax or Email to initiate your test kit being sent by mail.
