Patients in New York, Oregon, and Alaska  may request one or both of the following as related to their testing by Tempus and its affiliates, including Ambry Genetics: 

• Do not use my specimen or data for purposes beyond the testing ordered and what is required for clinical laboratory operations;
• Do not retain my specimen beyond the time required to complete the ordered testing or as required by applicable law.
