Portability of performance status calculations in real-world cancer care

This study explored the external validity of the Disability Score (DS), a measure developed to ascertain patient performance status (PS) when it is missing from real-world data. Using Tempus AI records and claims data, the study analyzed a cohort of 5,784 patients with metastatic breast, colorectal, and non-small cell lung cancer. While the DS model showed high sensitivity and accuracy, it had consistently low specificity, indicating that it overclassified patients with good PS. The study concludes that the DS measure was not externally valid in these real-world cohorts, highlighting the need for future PS imputation models to leverage datasets that more closely align with the target population.
